This Privacy Policy (“Policy”) describes how Your Signature Wellness (“YSW,” “we,” “us,” or “our”) collects, uses, and discloses information when you visit our website, contact us, or otherwise interact with us before you become an established patient.
This Policy is different from our HIPAA Notice of Privacy Practices. If you register as a patient and receive telehealth services from YSW, the health information you share with us as part of your care — your Protected Health Information (“PHI”) — is instead governed by our HIPAA Notice of Privacy Practices, which is provided to you separately and which you will be asked to acknowledge before receiving services. This website Privacy Policy covers general website activity and information collected before a patient relationship is established, such as browsing our site or submitting a contact form.
We reserve the right to revise this Policy at any time. Any changes will be posted on this page with an updated effective date. Continued use of our website after changes are posted constitutes acceptance of the revised Policy.
Information you provide to us, such as when you fill out a contact or intake-interest form, sign up for our newsletter, or email or message us: your name, email address, phone number, and any other information you choose to include.
Information we collect automatically, when you visit our website: your IP address and approximate location (city/state); browser type and device information; pages viewed and time spent on our site; the site that referred you to us; and similar technical and usage data.

Information from third parties, such as advertising platforms and analytics providers, when you arrive at our site through an ad or promotional link.
Our website uses cookies and similar tracking technologies, including advertising pixels from platforms such as Meta (Facebook/Instagram), TikTok, LinkedIn, and Google, to help us understand how visitors use our site and to measure and improve the performance of our advertising campaigns. These technologies may collect information such as pages visited, actions taken on our site, and general device/browser information, and may allow these advertising platforms to show you more relevant ads on their own platforms.
We do not knowingly permit these advertising technologies to collect Protected Health Information. Website analytics and advertising pixels are used only on public marketing pages, not within the secure patient portal.
You can control cookies through your browser settings, including blocking or deleting them; note that some parts of our website may not function properly if you disable cookies.
We use the information described above to: operate, maintain, and improve our website; respond to inquiries and provide requested information; send marketing communications about our services (you may opt out at any time, see Section 6); measure and improve the effectiveness of our advertising; comply with legal obligations; and protect against fraud, abuse, or security incidents.
We may share information described in this Policy with: service providers who help us operate our website, communications, scheduling, and marketing (such as our patient portal/EMR provider, email or SMS platform, and advertising and analytics partners); professional advisors (such as attorneys or accountants) as needed; and others, when required by law, to protect our rights, or in connection with a business transaction such as a merger or sale of assets.
We do not sell personal information collected through this website to third parties for their own independent marketing purposes.
Our website and services are intended for adults. We do not knowingly collect personal information through our website from children under 13, and our services are not directed at soliciting business from children. If you believe a child has provided us information through our website, please contact us using the information in Section 9 so we can address it.
You may opt out of marketing emails at any time using the unsubscribe link included in those emails, and may opt out of marketing text messages by replying STOP. You may also contact us directly (Section 9) to ask what information we have collected about you through our website, or to request that we delete it, and we will honor reasonable requests to the extent not otherwise required to be retained by law.
We retain website and contact-form information only as long as reasonably necessary for the purposes described in this Policy, or as required by law. Information that becomes part of your medical record once you become a patient is retained in accordance with our HIPAA Notice of Privacy Practices and applicable Florida medical records retention law (a minimum of five years from your last date of contact with YSW, per Fla. Stat. § 456.057).
We use Secure Socket Layer (SSL/TLS) encryption to protect information transmitted through our website, and we require our patient portal and electronic medical record platform to maintain administrative, technical, and physical safeguards appropriate to the sensitivity of the information involved. No method of transmission or storage is 100% secure, and we cannot guarantee absolute security.
